Returning home following a recent surgery or hospitalization often requires extra care. A smooth transition back into the home can help prevent hospital re-admittance and re-injury.
We will be in close contact with you, your family members, power of attorney (if applicable), and your doctor to assist in a speedy recovery.
The care involved for post-hospitalization combines all of our other services but it also includes:
-Scheduling doctors, rehabilitation or therapy appointments
-Alerting doctors, nurses or family members of any change in condition
-Providing medication reminders (but not supplying the medications directly)
-Picking up prescriptions or needed medical equipment
-Assisting in physical therapy routine (prescribed by a doctor or physical therapist)
–Nutritional management, meal prep, grocery shopping to accommodate any specific dietary needs/restrictions
-Assisting with transfers
-Bathing and hygiene
-Assistance with ambulation
-Tidying up at home, light housekeeping and laundry
–Transportation to appointments or to the pharmacy
-Being a medical witness at doctor’s appointments in the event a family member has any questions or concerns
